If your loved one passed away in North Dakota, you may order their death certificate online, by mail, or in person. Details for obtaining a death certificate in these different ways are below:

Online

To order a death certificate online, go to this website and follow the prompts. A form of payment (Visa, MasterCard, Discover) will be required. 

By Mail

To order a death certificate by mail, complete this application form and send it to the address below. Include a check or money order made out to the “North Dakota Department of Health” for all required fees. Also remember to include primary or secondary forms of identification.
