Request a death certificate online by visiting VitalChek which is an independent third-party entity. There are fees charged by VitalCheck up to $40 depending on the form you wish to receive your certificate in. 

2. By Mail

Request a death certificate by sending a completed certificate request form, photocopy of acceptable identification, and a signed check or money order for the certificate fee made out to Idaho Vital Records. Mail these items to the following address:
