After a loved one’s passing, you may need to deal with items such as driver’s licenses, license plates, and handicap placards that have specific rules governing how they are discarded. Because some of these items can be used toward identity theft, it is important you follow these rules.

Provide a copy of the death certificate, memorial card, or obituary notice to the DVS (Driver and Vehicle Services), which can be located via the contact information below. The DVS will flag the record holder as deceased.

If you have online access, please upload documentation to the Online Services site. Under Submit Forms, Upload Miscellaneous Forms, then click on Death Notices and follow the prompts.
